The Scarlet Empress founded and ruled the Realm.
She earned her right to rule by ending the Balorian Crusade with the Sword of Creation, now called the Realm Defense Grid. She entered the manse controlling the worldwide weapon network, and came out the only one of her group still alive. She used her fame and the threat of turning the Sword against her enemies to found the Realm and consolidate power, and has been ruling a large portion of the world ever since... until five years ago.
In Realm Year 763, during Calibration, the Scarlet Empress simply disappeared. Nobody is certain what's become of her, but with no clear line of succession in place, the Great Houses are in disarray and on the verge of civil war.
